2008-2009 Lottery Recipients


Please familiarize yourself with the registration instructions and forms by visiting the links below. Documentation must be included with each sibling/public registration application in order for the application to be complete and issued a registration number.  Therefore, it is important for you to read completely the registration packet so that you comply with all requirements.  Please determine the appropriate registration dates and times before you register.  Thank you!

Current Student Registration:                                                                     February 12-16, 2008 posted

Public Registration Period*:                                                                      March 10 - March 27, 2008 posted

Posting of lottery recipients on the internet:                                                April 11, 2008 @ 5 p.m. posted

Deadline for receipt of acceptance letter from lottery recipients::                 April 18, 2008 @ 5 p.m.

Wait List Registration:                                                                                Begins April 21, 2008 (school days 8:30 a.m. - 1 p.m.)
 
 
*In the event that registration for a particular grade level exceeds the number of seats available for that grade the registration will go to lottery.  Lottery is where all registrant names are placed in a box and the number of available seats are pulled.  All remaining names are then placed on a wait list for that grade level. (This event could occur during sibling or public registration.)

Registration Rules and Requirements:

Please note that completed registration forms, including all supporting documentation, will be accepted school days between the hours of 8:30 a.m. - 4 p.m.  Registration can only be completed in person as all documentation must be provided at time of registration in order to receive a lottery number. Registration applications will not be accepted without the following forms attached at the time of walk-in registration for any person who is not currently a student at Somerset (this includes siblings of current students):

q       School Board of Broward County, Florida School Registration Form (1 page) (forms are attached in English, Spanish, Creole, & Portuguese - complete form in parent home language)

q       Somerset Academy Registration Application (Application, Registration Rules and Commitments, Student Code of Excellence, Parent Contract – 6 pages)

q       Somerset Academy Student Contract (1 page)

q       Somerset Academy Uniform Policy (1 page)

q       Broward County Public Schools Code of Student Conduct (1 page)

q       Somerset Academy Parent Compact (1 page)

q       Somerset Academy Volunteer Policy (1 page)

q       Copy of Birth Certificate

q       Two Proofs of Broward County Residence (see below)

q       Original Health and Immunization Forms (Yellow & Blue – due at time of registration or within one week of the child reading the age of 5 for students entering Kindergarten)

q       Copy of 2nd Quarter 2007-2008  Report Card (for students entering first through fifth grade)

q       Copy of current health insurance card

q       Copy of final report card to be delivered to the Somerset Academy campus registered no later than June 30, 2008

Age Requirement 

Kindergarten:                                                  Children who will be 5 years old on or before September 1, 2008 shall be eligible for admission to kindergarten

First Grade:                                                  ·  Children who will be 6 years old on or before September 1, 2008 and who have been enrolled in a public school
                                                                       shall be eligible for admission to first grade.

                  ·  Students who have completed kindergarten in a private school AND who are 6 years old on or before September 1,
                      2007 shall be eligible for admission to first grade.  A letter from the private school stating satisfactory completion of
                      kindergarten must be presented at time of registration.

ADDITIONAL REQUIRED DOCUMENTATION FOR ALL NEW REGISTRANTS (including siblings):

PROOF OF BROWARD COUNTY RESIDENCE (Domicile)

One current piece of evidence from the following sources in the name of the parent/guardian registering the child

_____ Current telephone or electric bill in the name of the parent registering the child.
_____ Rent receipt with the name of lessor and contact information/ mortgage statement.
_____ Lease agreement with name of lessor and contact information.
_____ Mortgage commitment.
_____ Home Purchase contract including specified closing date, with copy of deed to be provided within 60 days of closing date

AND - One of the following:
_____ Automobile insurance.
_____ Current Florida Driver's License/Florida Identification Card.
_____ Cellular telephone bill.
_____ Credit card statement.
_____ Bank account statement.
_____ United States Postal Service confirmation of address change request or evidence of correspondence delivered though U.S. postal service.
_____ Declaration of Domicile form from the County Records Department.

HEALTH & MEDICAL RECORDS

Original Health and Immunization records (Blue & Yellow) must be presented at the time of registration. The proper form needed to document a student's immunization history is the Department of Health Form 680.  The Florida Plan for School Health Services requires that all students (PK-12) submit documentation of a Student Health Examination performed within the 12 months prior to initial entry into a Florida school.  A Student Health Examination (DH or HRS-H Form 3040), including proof of tuberculin skin test, reading of test, and appropriate follow-up, should be completed and signed by a licensed practicing health care provider, and presented to the school at the time of registration.
